Purpose of this notice: To ensure users of certain nucleic acid testing (NAT) technologies are aware of certain aspects of the instructions for use (IFU) for all products.
Description of the problem: WHO has received user feedback on an elevated risk for false SARS-CoV-2 results when testing specimens using RT-PCR reagents on open systems.
As with any diagnostic procedure, the positive and negative predictive values for the product in a given testing population are important to note. As the positivity rate for SARS-CoV-2 decreases, the positive predictive value also decreases. This means that the probability that a person who has a positive result (SARS-CoV-2 detected) is truly infected with SARS-CoV-2 decreases as positivity rate decreases, irrespective of the assay specificity. Therefore, healthcare providers are encouraged to take into consideration testing results along with clinical signs and symptoms, confirmed status of any contacts, etc.
Users of RT-PCR reagents should read the IFU carefully to determine if manual adjustment of the PCR positivity threshold is necessary to account for any background noise which may lead to a specimen with a high cycle threshold (Ct) value result being interpreted as a positive result. The design principle of RT-PCR means that for patients with high levels of circulating virus (viral load), relatively few cycles will be needed to detect virus and so the Ct value will be low. Conversely, when specimens return a high Ct value, it means that many cycles were required to detect virus. In some circumstances, the distinction between background noise and actual presence of the target virus is difficult to ascertain. Thus, the IFU will state how to interpret specimens at or near the limit for PCR positivity. In some cases, the IFU will state that the cut-off should be manually adjusted to ensure that specimens with high Ct values are not incorrectly assigned SARS-CoV-2 detected due to background noise.
Manufacturers regularly review the design of their product, including labelling and IFU based on customer feedback. In the early phases of the COVID-19 pandemic, in vitro diagnostics (IVDs) were rapidly developed, validated and verified, and then rolled out. Therefore, it is not unexpected that IVDs may require refinement based on user feedback after their introduction at scale. Users should verify the version of the IFU with each consignment they receive to see if any changes have been made to the IFU.

Anyway, this 90 year old lady is a crisis actor, and this whole story is just 'theatre' for the media. Its difficult to judge the age of the 'daughter' and 'grandson' because they're both wearing face coverings (how convenient), but I agree it does look odd.
7 year old daughter went to spec savers for an eye test last week. Usual bullshit invented prescription for money making. Insisted she wear a mas k as it was company policy. Meant to get glasses next week sometime.

ReplyDeleteI'm having some thoughts I'd like to brain dump...probably rhetorical but I dont really have anyone to talk to....
Its being reported most of the positive cases are high school kids (makes sense after they started testing high school kids???!!) Well then why arent our teachers dropping like flies???
Which also makes me think why arent our police, nurses, drs, supermarket workers dropping like flies???
I have school age children so I now know lots of people who have tested positive - most no to mild symptoms a couple were really quite poorly but didnt need hospital care - why isn't this making anyone cynical about the restrictions because practically everyone is recovering from this "virus".
The hospitals are reported as overwhelmed but also this will be due to isolation periods surely and staff having to go off sick? See I worked for the NHS for 14 years and the only reason it was ok not to go to work was if you were dead. And I'm dead serious - they came down hard on sickness. The surgeons I worked with would come in cramping up from the flu each winter, nasal drip everywhere, knackered and ill and still perform 8 hour surgery on patients who had cancer (so very poor immune systems) so what gives there. Why was it ok to put staff and patients at risk every single day then because they just simply couldn't cope if you went off sick but now there is no mention of staffing levels? Are they overwhelmed with more patients? Or are they overwhelmed with less staff??

There's a widespread misconception that humans as a species are 'evolving' with each generation to enjoy longer life expectancy: and that this is indicative of (among other things) massively better nutrition plus huge advancements in medicine and healthcare -- resulting in 'healthier bodies'.

In fact, throughout history and across the world, there have *always* been humans who lived in to their 90s, to 100, or beyond.
So why does it appear from the life expectancy statistics that humans (at least in the West) are somehow 'evolving', physiologically, to have sturdier and more durable bodies -- 'longer lasting'?
It's largely because of the longterm decline in babies and infants dying at birth or in the first few years of life.
Childhood diseases which were once common have now been largely eradicated from the West, or no longer culminate in death in most cases.
The result of the falling infant and child mortality rate has been a steady increase in the *average* age of people at death, creating, in the minds of many, the *false* impression that human bodies are somehow 'evolving' to last longer with each generation.
Why is this important?
Because, related to all this is a misconception that old people in the West are healthier today than ever before, and living healthier, happier lives.
No they are NOT! In some cases, sure. But not typically.
In many ways, a typical 80 or 90 year-old in the West today is in a considerably *poorer* state of health than an 80 or 90 year-old from a past agricultural era (or an agricultural society of today).
In this era in the West it is rather common for senior citizens to be reliant on ingesting *dozens* of medications, every day, to treat the symptoms of a large numbers of ailments and serious health conditions.
For in today's world, many elderly (and many not-so-elderly) folk in North America, Western Europe, Australia and other post-industrial societies suffer terribly from severely reduced mobility, low physical stamina, poor eyesight and hearing, weak hearts, food intolerances and/or allergies, susceptibility to infections, cancers, diabetes, etc
Many of these ailments and health conditions are and have been caused by our environments, our levels of activity or non-activity, our habits of mind and body, and the foods we eat or do not eat.

But modern medicine and healthcare cannot fully undo the damage caused to us by *decades* of eating highly processed, low-nutrition foods... relying on motorised transport instead of walking and exercising... ingesting toxic fumes and waves from industry, household and personal chemicals, and mobile phones/transmitter towers... damaging our eyesight through staring at screens and pages vs engaging with the far horizon in nature... worrying/fearing/becoming unhappy about things we cannot change or affect, caused by mass media consumption, consumer culture, and the over-abundance of trivial and meaningless consumer 'choices'... stress caused by living alienated from trees, plants, animals and the soil.. unhappiness caused by living at some distance from our extended family, and friends who live far away... stress caused by feelings of personal inadequacy or 'incompleteness' caused by pointlessly comparing ourselves to others... the lack of a sufficient number of 'meaningful' jobs available in our economies, as they are presently constituted, with the proliferation instead of 'drone' jobs in call centres and warehouses...
And once could go on.
In summary, we are not "living longer, healthier lives," except to the limited extent that most of us do not die during childbirth or before the age of, say, 10 years old.
But we in the West *could* enjoy much healthier lives in old age, if only we adopted healthier habits of body and mind in childhood and practiced them throughout the decades. This will not be possible until we reform the current capitalist system, which prioritises profits, and the desires of billionaires, over the health and happiness of 99.9% of ordinary people.
Aang.....just got my Freedom of Information back from NHS Highland regarding Covid deaths

they weren't specific enough and only gave me a total of 'mentioning Covid on the death certificate'....clearly this is not defined enough from my request but let's just go with it for now, we can also assume like England/Scotland etc that being non specific means approx 10% died from Covid whilst the 90% died but tested positive with Covid
anyhoo, their answer is
"There have been 131 deaths registered of NHS Highland residents mentioning Covid-19 on the death certificate to date"
Now, depending what site you look at the population of Highlands (not including Islands etc) is listed as 235,540 in 2018
So the total percentage of ALL listed from and with Covid is 0.0556% (remember that's all cases with COVID 'listed', not necessarily 'from', on the death certificate, many of them could have died of cancer, heart attacks, road collisions etc etc)
so up here we have a 99.9444% of surviving it, if we get it, if it even exists in the first place
They put us in Tier 4 after being Tier 1 most of these 'Tier' times and full lockdown beckons after Boxing Day
